Background
The
Michigan Acute Stroke Care Overview & Treatment
Surveillance System
(MASCOTS)
is a representative, statewide, hospital-based
surveillance system of acute stroke care in Michigan.
MASCOTS is a multi-center project based at Michigan
State University that involves collaboration with two
other major universities (University of Michigan and
Wayne State University), the Michigan Department of
Community Health, and a total of sixteen hospitals
across the state. The primary objective is to develop
and refine the methods and infrastructure necessary to
conduct long-term surveillance of acute stroke care and
stroke outcomes in the state of Michigan.

Michigan is one of only 4 states that have been funded
by the Centers for Disease Control and Prevention as
part of the Paul Coverdell National Acute Stroke
Registry. The intent of this 12-month federal grant
is to design and pilot test a mechanism to collect data
concerning the delivery of care to acute stroke
patients, with the ultimate goal of reducing mortality
and disability from stroke through improved stroke care.

MASCOTS involves the collection of individual-level
patient information including demographics, pre-hospital
EMS information, emergency department triage,
in-hospital stroke, imaging results, in-hospital
procedures, treatments and complications, medical
history and risk factors, secondary prevention measures,
final diagnosis, and discharge status.

The process of identifying and tracking acute stroke
patients will first be pilot-tested in 8 hospitals from
4 regional sites (Detroit, Washtenaw County, Ingham
County, and Kalamazoo). Following this pilot phase,
MASCOTS will be implemented statewide in all 16
hospitals for at least a 6-month period.

Findings from MASCOTS will be distributed through
established public health mechanisms to important
stakeholders - including the Michigan Stroke Initiative,
American Heart Association, Operation Stroke Sites, the
Michigan Hospital Association, Medicaid and Medicare,
and Managed Care Organizations. This project represents
a unique opportunity for change in the standards of care
for acute stroke patients in Michigan and ultimately
across the country as a whole.
